Working Papers Reminder

1. Working papers must have a handwritten signature in ink. Documents cannot be photo copied or faxed.

2. Working papers must have a parent / guardian signature.

3. Physical Fitness Certification form must be signed by a physician with an address or an address stamp included.

4. Student must return the working papers and physician certificate in person to the Guidance office as the actual working permit must be signed in the presence of the issuing officer.

5. If you are not a PPCS District student, you must present a raised seal birth certificate for proof of age.
